Adult dose mistakenly given to child during pediatric rollout

An adult dose of COVID-19 vaccine was mistakenly administered to an 11-year-old child in Manila.

The incident occurred at a pediatric vaccination site where the child's mother had brought her daughter for a second dose of Pfizer vaccine.

According to the child's vaccination card, she received her first dose on January 10.

DOH-NCR is currently investigating this error which happened during the initial rollout of vaccinations for children aged 5-11 that began in Manila on February 7.

Meanwhile, Manila vaccinated 555 children aged 5 to 11 with a reformulated Pfizer vaccine on Monday, marking the start of the 'Rebakuna Kids' program.
